Cutler on testing thyroids:
You actually want TSH, T3, T4, free T3 and free T4, not just whatever the lab feels like doing when he puts down "thyroid."
From a thread called Adrenal Fatigue vs. Low Thyroid Function:
The adrenal function has to be about 80 % gone or more before the person gets to the nausea, vomitting, severe weight loss stage. Most people with chronic fatigue/fibro haven't got that far yet. Also, low thyroid and inactivity due to pain and fatigue will contribute to weight gain. In general people with chronic fatigue are low in cortisol.
The Metabolic Scorecard: Symptom Matrix may help differentiate between adrenal fatigue and low thyroid function.
Cutler's Hair Test Interpretation book mentions that opposing levels of Ca/Mg and Na/K (e.g. high calcium, high magnesium, low sodium, low potassium, OR low calcium, low magnesium, high sodium, high potassium) may indicate adrenal fatigue. He also states that medical texts acknowledge that it is difficult to diagnose adrenal fatigue through other methods until it is severe enough to cause death.
